Job Description:
Glen Cove Hospital is a 265 bed hospital and is home to the Orthopedic and Rehabilitation Institute, which is internationally known with the state of the art programs in joint replacement and an all inclusive spine program. The Wunsch Center for Rehabilitative Therapies is among the top rated facilities in the New York metropolitan area. The hospital also offers a 44 bed Geriatric Unit treating older adults with a full spectrum of health problems.
We are currently looking for a part-time Social Worker for the Family Medicine Department to work collaboratively with a team of Social Workers and RN/Discharge Planners. The ideal candidate will perform psycho-social assessments, individual/family counseling, adjustment-to-Illness counseling, crisis intervention, and discharge planning services. Group Work maybe also be entailed.
This position is 18.75 hours per week
Qualifications
*
Master's degree in social work, along with Licensure (LMSW, LCSW) required
*
Bilingual in English/Spanish required
*
Knowledge of community programs and other resources in the area of North Shore Long Island
*
Experience with patients from Prenatal through Geriatrics
